Photo and Media Release:

Arts for Learning NW will take documentation of the camp program for promotion, to share with camp families and with PPS.

By clicking yes, I hereby consent to have my child photographed, videotaped, and interviewed as part of their participation in the ALNW Camp. I also consent to ALNW’s and its partners’ use of my student’s photograph, likeness, voice or creative works in any type of media. I agree to release and hold harmless ALNW, its agents, employees and contractors from and against any and all claims or other forms of liability that shall arise by reason of the use of participant’s likeness, photograph, voice or creative works. It is further understood and I do agree that no monies or other consideration in any form will be provided because of any of the above activities or the above-described use of my student’s photograph or likeness, voice or creative work(s).

By clicking no I indicate that I do not consent to my student being photographed, videotaped, audio taped by Arts for Learning NW.

Waiver of Liability

I hereby give permission to Arts for Learning NW (ALNW) for my camper to take part in ALNW Summer Camp program (Camp)

I understand that every reasonable effort will be made to ensure the safety of the Camper.

I understand and agree that in the event of an emergency injury or illness, ALNW staff members will first phone listed emergency contacts and in the event that a response is not received within an hour of call, ALNW staff in charge of Camp may act on my behalf and at my expense in obtaining medical treatment for my child. I hereby waive, release and hold harmless ALNW, its agents, its sponsors and any and all persons and organizations associated with Camp from any and all injuries, negligence, damage and for any claims or causes of action arising from Participant’s connection to Camp. I will be financially responsible for any medical and/or other charges in connection with the Participant’s time in Camp.

I understand that there is health risk in sending Camper to Camp including but not limited to the COVID-19 virus. I agree not to send Camper to Camp if they exhibit any signs of COVID-19 virus, or if they test positive to COVID-19. I will not hold ALNW liable for illness or exposure of COVID-19 or any other health risk as related to Camper’s time in Camp.

I understand that ALNW may choose to cancel one or more days of Camp if we cannot ensure a safe camp environment because of excessive heat, sickness or other unsafe situations.

I understand that Camper will not be released at the end of camp to any adult without an ID that matches the name of an approved adult on file. I understand that Participant needs to be picked up from camp in a timely fashion and agree to call ahead if I know I will be late for any reason. If approved adults are more than 10 minutes late we will call all approved pick-up adults. If Participant is not picked up and no notice has been given to camp staff, within 30 minutes of the end of class we will be forced to contact law enforcement.
