Photo and Media Release:

Arts for Learning NW will take documentation of the camp program for promotion, to share with camp families and with PPS.

By clicking yes, I hereby consent to have my child photographed, videotaped, and interviewed as part of their participation in the ALNW Camp. I also consent to ALNW’s and its partners’ use of my student's photograph, likeness, voice or creative works in any type of media. I agree to release and hold harmless ALNW, its agents, employees and contractors from and against any and all claims or other forms of liability that shall arise by reason of the use of participant’s likeness, photograph, voice or creative works. It is further understood and I do agree that no monies or other consideration in any form will be provided because of any of the above activities or the above-described use of my student's photograph or likeness, voice or creative work(s).

By clicking no I indicate that I do not consent to my student being photographed, videotaped, audio taped by Arts for Learning NW.

Waiver of Liability

I hereby give permission to Arts for Learning NW (ALNW) for my camper to take part in ALNW Summer Camp program (Camp)

I understand that every reasonable effort will be made to ensure the safety of the Camper.

I understand and agree that in the event of an emergency injury or illness, ALNW staff members will first phone listed emergency contacts and in the event that a response is not received within an hour of call, ALNW staff in charge of Camp may act on my behalf and at my expense in obtaining medical treatment for my child. I hereby waive, release and hold harmless ALNW, its agents, its sponsors and any and all persons and organizations associated with Camp from any and all injuries, negligence, damage and for any claims or causes of action arising from Participant’s connection to Camp. I will be financially responsible for any medical and/or other charges in connection with the Participant's time in Camp.

I understand that there is health risk in sending Camper to Camp including but not limited to the COVID-19 virus. I agree not to send Camper to Camp if they exhibit any signs of COVID-19 virus, or if they test positive to COVID-19. I will not hold ALNW liable for illness or exposure of COVID-19 or any other health risk as related to Camper’s time in Camp.

I understand that ALNW may choose to cancel one or more days of Camp if we cannot ensure a safe camp environment because of excessive heat, sickness or other unsafe situations.

I understand that Camper will not be released at the end of camp to any adult without an ID that matches the name of an approved adult on file. I understand that Participant needs to be picked up from camp in a timely fashion and agree to call ahead if I know I will be late for any reason. If approved adults are more than 10 minutes late we will call all approved pick-up adults. If Participant is not picked up and no notice has been given to camp staff, within 30 minutes of the end of class we will be forced to contact law enforcement.

Arts for Learning NW tomará la documentación del programa del campamento para su promoción, para compartir con las familias del campamento y con PPS. Al hacer clic en Sí, doy mi consentimiento para que mi hijo sea fotografiado, grabado en video y entrevistado como parte de su participación en el Campamento ALNW. También doy mi consentimiento para que ALNW y sus socios usen la fotografía, la imagen, la voz o los trabajos creativos de mi estudiante en cualquier tipo de medio. Al hacer clic en no, indico que no doy mi consentimiento para que mi estudiante sea fotografiado, grabado en video o grabado en audio por Arts for Learning NW.

Exoneración de responsabilidad

Al registrar a mi hijo en el campamento de verano de Arts for Learning NW, doy permiso a Arts for Learning NW (ALNW) para que mi campista participe en el programa de campamento de verano de ALNW (campamento) Entiendo que se harán todos los esfuerzos razonables para garantizar la seguridad del campista. Entiendo y acepto que en caso de una lesión o enfermedad de emergencia, los miembros del personal de ALNW llamarán primero a los contactos de emergencia enumerados y, en caso de que no se reciba una respuesta dentro de una hora de la llamada, el personal de ALNW a cargo del campamento puede actuar en mi nombre y por mi cuenta para obtener tratamiento médico para mi hijo. Por la presente renuncio, libero y eximo de toda responsabilidad a ALNW, sus agentes, sus patrocinadores y a todas y cada una de las personas y organizaciones asociadas con el Campamento de cualquier lesión, negligencia, daño y cualquier reclamo o causa de acción que surja de la conexión del Participante con el Campamento. Seré financieramente responsable de cualquier cargo médico y/o de otro tipo en relación con el tiempo del Participante en el Campamento. Entiendo que existe un riesgo para la salud al enviar a Camper a Camp, incluido, entre otros, el virus COVID-19. Acepto no enviar a Camper al Campamento si presenta algún signo del virus COVID-19, o si da positivo a COVID-19. No responsabilizaré a ALNW por enfermedad o exposición a COVID-19 o cualquier otro riesgo para la salud relacionado con el tiempo del campista en el campamento. Entiendo que ALNW puede optar por cancelar uno o más días de Campamento si no podemos garantizar un ambiente de campamento seguro debido al calor excesivo, enfermedad u otras situaciones inseguras. Entiendo que Camper no será entregado al final del campamento a ningún adulto sin una identificación que coincida con el nombre de un adulto aprobado en el archivo. Entiendo que el participante debe ser recogido del campamento de manera oportuna y acepto llamar con anticipación si sé que llegaré tarde por algún motivo. Si los adultos aprobados llegan más de 10 minutos tarde, llamaremos a todos los adultos aprobados para recogerlos. Si no se recoge al Participante y no se notifica al personal del campamento, dentro de los 30 minutos posteriores al final de la clase, nos veremos obligados a comunicarnos con la policía.
